TITLE: Residential Program Worker/Trainee

SUPERVISED BY: Residential Program Supervisor

QUALIFICATIONS: Must be at least 18 years of age with:

  1. High School Diploma or GED
  2. Valid PA Driver's License and Insurable with the Agency's Motor Vehicle

Policy

  1. Satisfactory Physical Examination In accordance with DPW Regulations with ability to perform duties outlined In the Job Description
  2. Pennsylvania Criminal History Clearance
  3. FBI Criminal History Clearance for Employees residing outside Pennsylvania

PHYSICAL REQUIREMENTS: A () indicates requirements specific for the Individuals employee will be working with.

1. Assist and/or support Individual with Impaired mobility entering and exiting vehicle, both tub, stairs, etc.

2. Physically control/restrain an aggressive or combative individual UP TO _______150 LBS. _______ UP TO 200 LBS ________UP TO 300 LBS

3. Quick action and mobility to redirect a fleeing individual to safety.

4. Ability to use upper extremities for bands-on sign language.

5. Drive a van carrying up to 9 individuals in all weather conditions including night driving.

6. Lift and carry loads weighing _____ 5 -20 lbs. _____ 20 - 50 lbs.
_____ 50 - 70 lbs. _____ 75 - 100 lbs.

7. Standing and/or walking ____up to 15 minutes ____ 30 minutes
______1 hour _____1 hour or more.

8. Climb and descend stairways _____ up to 1 flight _____ I flight or more

9. Operation of common household appliances including a microwave.

10. Snow removal and lawn maintenance.

11. Adaptation to shift work Including awake nights and quick rotation between different shifts.

12. Ability to cope with the mental and emotional stress of the position.

13. Adequate hearing and eyesight to assure individual's safety.

14. Absence of any medical problem including communicable disease that might interfere with the health, safety, or well-being of the individuals unless all requirements of DPW regulations 151 and 152 are met.

CORE RESPONSIBILITIES

  1. Appropriately assist and teach daily living routines in the following areas:
    1. Personal hygiene
    2. Food preparation
    3. Care or personal and common living space
    4. Care of laundry and clothing
    5. Developing daily routines
  1. Implementation of IPP (Individualized Program Plan)
    1. Implement goal plans 93 written
    2. Provide appropriate suggestions for IPP
    3. Follow plan for social, emotional, and environmental supports
    4. Follow plan for participation In community life
    5. Update likes/dislikes list as needed
    6. Complete appropriate assessments as assigned
    7. Complete bi-weekly summaries as assigned
  2. Facilitate community integration/advocacy
    1. Actively encourage appropriate participation In the community
    2. Actively look for activities the individual would enjoy
    3. Present Individual in a positive manner to the community
    4. Respond to Individual's requests and grievances according to policies and procedures
  1. Maintenance or residence
    1. Maintain petty cash, long distance phone call log, and other household records appropriately
    2. Keep home environment clean and comfortable
    3. Perform routine household repairs and maintenance
    4. Reports any maintenance needs to supervisor
    5. Spend agency funds appropriately when using petty cash, food accounts, and telephone (including cellular)
    6. Follow fire safety regulations
  1. Assist with individual financial needs
    1. Accurately records entries on Individual's monthly financial record, receipt record, personal property Inventory, and bank accounts.
    2. Maintains accurate balances of Individual's cash on hand, checking, savings, and food stamps
    3. Assists Individual in monthly budgeting according to Financial IPP
    4. Prevents misuse or theft of Individuals' personal and financial property by anyone.
    5. Addresses individual's financial needs with supervisory staff
    6. Assist individuals with personal shopping as appropriate and checking prices while honoring individual preferences.
  1. Assist with Individual medical needs
    1. Administers and records medications correctly
    2. Monitors individual's health recognizing the appropriate level of care needed for the situation
    3. Appropriately assists Individual during medical appointments
    4. Assures medical forms are completed when necessary
    5. Follows universal precautions
  1. Interact with consumers, co-workers, families and management In positive, supportive and cooperative ways
    1. Actively seeks direction from supervisor when needed
    2. Follows agency policy and procedures
    3. Treats Individuals and families respectfully
    4. Maintains confidentiality
    5. Teach new staff as assigned by supervisor
    6. Communicate positively and professionally with team members or co-staff regarding household, programming and agency concerns
  1. Maintenance of agency vehicles
    1. Perform vehicle maintenance checks as assigned
    2. Reports mechanical and maintenance problems In a timely manner
    3. Maintains interior and exterior care of vehicle as assigned
    4. Operates vehicles to avoid undue wear and unnecessary repairs
    5. Maintains accurate mileage report records
  1. Operation or agency vehicle
    1. Drives safely, obeying all traffic laws
    2. Assures Individuals are stated safely and secured In seat belts
    3. Follows any guidelines for seating arrangements for Individuals
    4. Follows and understands driver selection policy
  1. To be Dependable
    1. Assures DPW preservice and interservice trainings are received by attending all scheduled trainings and passing any required tests.
    2. Attends bi-weekly staff meetings or other scheduled meetings
    3. Reports to work on time and on days scheduled
    4. Submits vacation requests promptly and according to policy
    5. Uses sick time appropriately and reports off according to policy
    6. Flexible In scheduling required In a 24 hour facility
